Please fill out the following form to request tickets for a customer. Requests will be reviewed and confirmed with Jeff as they are received, and must meet the following criteria:

-Reason for invitation has been discussed and approved by your manager*
-Guests are all adults, age 21+

AND
-Guest is in within our Top customer list, or
-Guest is a significant opportunity for wider/deeper, or
-Guest is a significant NEW business opportunity, or
-Guest is a qualified Production or MITS prospect

*If client does not meet the above criteria additional approval from Jeff Elkin, Tim West or Bill Siciliano is required.

You will receive a separate confirmation email from Kate Fenwick for each customer if your request has been approved.
